Comprehensive medication review (CMR)

Taking prescription medicines can be a challenge. There are a lot of details to understand. One member of your health care team - a pharmacist practitioner - can help you manage your medicines and your health. A pharmacist practitioner is a doctor of pharmacy (PharmD) that is specially trained to provide patient care in the clinic setting.

Our goal is to improve quality of life by ensuring that medications are appropriate, effective, safe, and convenient.

Our Pharmacist Practitioners work with your providers, educate about medications and conditions, and adjust medications to maximize their effect. We take action to correct drug therapy problems and follow up to ensure a resolution. We combine clinical evidence and personal preference with the goal of empowering people to make informed decisions about their medications and about their health.

Pharmacist Practitioners are available at multiple Allina Health Clinic locations. To schedule a private appointment, call 612-262-3224 or schedule online through your Allina Health Account and select 'Pharmacist Medication Review'.

What to expect

A visit with the pharmacist practitioner may help you save money on medicines. An internal Allina Health report shows that we can help you save an average of $230 annually on medications (or $19 a month).

During the review, the pharmacist will:

  • tell how safe your medicines are
  • tell how well your medicines are working
  • assess any needs or problems caused by your medicines
  • create a care plan just for you
  • give suggestions to your regular health care provider
  • schedule a follow-up appointment (if needed)
  • answer your questions
  • Medication Therapy Management (MTM) services

Bring all of the medications you are taking. Include prescription, over-the-counter, vitamins, herbals and natural medicines. The pharmacist practitioner needs to know all of the medicines you are taking.

Also, bring a list of questions. For example, do you know what to do if you forget to take your diabetes pills, is there an herbal you want to start taking, or do you think your asthma inhaler isn't working as well as it used to
