An assessment allows you and your provider to create a rehabilitation plan that includes strategies to help you return to your best daily living.

What to expect

At the consultation, your provider will typically interview you, review your medical records, and administer standardized tests to measure cognitive abilities such as memory, language and problem solving. Your consultation may also take into account mood, personality and any disabilities you may have.

A referral from your physician is required.
