Comprehensive medication review (CMR)

PHARMACISTS HAVE AN IMPORTANT ROLE IN YOUR CARE

Taking prescription medicines can be a challenge. There are a lot of details to understand. One member of your health care team - a pharmacist practitioner - can help you manage your medicines and your health. A pharmacist practitioner is a doctor of pharmacy (PharmD) that is specially trained to provide patient care in the clinic setting.

Our goal is to improve quality of life by ensuring that medications are appropriate, effective, safe, and convenient.

Our Pharmacist Practitioners work with your providers, educate about medications and conditions, and adjust medications to maximize their effect. We take action to correct drug therapy problems and follow up to ensure a resolution. We combine clinical evidence and personal preference with the goal of empowering people to make informed decisions about their medications and about their health.

Several Allina Health clinic locations offer a special clinic visit that lets you talk privately with a pharmacist practitioner about your medicines.

A visit with the pharmacist practitioner may help you save money on medicines. An internal Allina Health report shows that we can help you save an average of $230 annually on medications (or $19 a month).

What to expect

During the review, the pharmacist will:

  • tell how safe your medicines are
  • tell how well your medicines are working
  • assess any needs or problems caused by your medicines
  • create a care plan just for you
  • give suggestions to your regular health care provider
  • schedule a follow-up appointment (if needed)
  • answer your questions.

Bring all of the medications you are taking. Include prescription, over-the-counter, vitamins, herbals and natural medicines. The pharmacist practitioner needs to know all of the medicines you are taking.

Also, bring a list of questions. For example, do you know what to do if you forget to take your diabetes pills, is there an herbal you want to start taking, or do you think your asthma inhaler isn't working as well as it used to.

"Highly reassured with this visit. Emily Hein left nothing unturned. She gave recommendations personal to my conditions. Love the mailing of new medication list and letter detailing every recommendation."

"Jill Konstantinides made recommendations that I brought to my neurologist who is outside of Allina Health. My neurologist was very impressed with the suggestions and implemented all of them. I finally have hope that my migraines will slow down."

"I greatly appreciate having access to Matt Kresl as the pharmacist on my care team. He has worked with me, my primary doctor, my diabetes educator and my mental health provider to ensure my medications work optimally for me and my health conditions."
