What it is

We offer short-term medical care for people in transitional care. Our teams work in transitional care units located throughout the greater Twin Cities area.

What to expect

An experienced nurse practitioner and a physician will visit you onsite at the transitional care unit. A nurse is available 24 hours a day, seven days a week.

Your Senior Health medical team will help to:

  • manage your medical care, rehabilitation and progress after your hospital stay
  • coordinate your care using Allina Health electronic medical records
  • help you overcome health barriers to provide a safe transition home
  • communicate with your primary care doctor and other home services to make sure you get the care you need when you leave the transitional care unit

 

Good to know

Services are billed to your insurance company, including Medicare. We will submit a claim for services on your behalf. Please contact your insurance carrier for questions about your insurance benefits.

Our short-term transitional care services are closely aligned with Genevive, a leading senior health care provider. We collaborate with Genevive’s transitional care unit teams, working together to advance care.

This is a retail clinic location. We offer basic services for minor injuries and illnesses for patients 18 months and older. We do not see visits for workers' or injury compensation.

Everyday On Wheels

Everyday On Wheels mobile clinic is not wheel chair accessible due to the size of the vehicle. Patients should be able to safely climb a short set of stairs to enter the mobile clinic.

Please arrive 10 minutes prior to your scheduled appointment time and have your mobile phone available for the provider to contact you when they are ready to see you.

All non-essential/non-urgent clinic visits are suspended until further notice. Use online scheduling only for essential or urgent visits.

If you have symptoms of coronavirus (fever, cough, shortness of breath or other flu-like symptoms), do not use online scheduling. Please isolate yourself at home and complete our free, 24/7 online coronavirus screening interview. We will provide you with next steps based on your symptoms.
