Short-term care in transitional care units

Bridging the gap from hospital to home

After a hospital stay for surgery or to treat an illness, you may not be quite ready to go home. Short-term care in a transitional care unit will help to keep a close watch on your medical condition as you regain strength and continue healing.

Overview

We offer short-term medical care for people in transitional care. Our teams work in transitional care units located throughout the greater Twin Cities area.

What to expect from your visit

An experienced nurse practitioner and a physician will visit you onsite at the transitional care unit. A nurse is available 24 hours a day, seven days a week.

Your Senior Health medical team will help to:

  • manage your medical care, rehabilitation and progress after your hospital stay
  • coordinate your care using Allina Health electronic medical records
  • help you overcome health barriers to provide a safe transition home
  • communicate with your primary care doctor and other home services to make sure you get the care you need when you leave the transitional care unit
