Short-term care in transitional care units
After a hospital stay for surgery or to treat an illness, you may not be quite ready to go home. Short-term care in a transitional care unit will help to keep a close watch on your medical condition as you regain strength and continue healing.
What it is
We offer short-term medical care for people in transitional care. Our teams work in transitional care units located throughout the greater Twin Cities area.
What to expect
An experienced nurse practitioner and a physician will visit you onsite at the transitional care unit. A nurse is available 24 hours a day, seven days a week.
Your Senior Health medical team will help to:
- manage your medical care, rehabilitation and progress after your hospital stay
- coordinate your care using Allina Health electronic medical records
- help you overcome health barriers to provide a safe transition home
- communicate with your primary care doctor and other home services to make sure you get the care you need when you leave the transitional care unit
