Overview

We offer short-term medical care for people in transitional care. Our teams work in transitional care units located throughout the greater Twin Cities area.

What to expect from your visit

An experienced nurse practitioner and a physician will visit you onsite at the transitional care unit. A nurse is available 24 hours a day, seven days a week.

Your Senior Health medical team will help to:

  • manage your medical care, rehabilitation and progress after your hospital stay
  • coordinate your care using Allina Health electronic medical records
  • help you overcome health barriers to provide a safe transition home
  • communicate with your primary care doctor and other home services to make sure you get the care you need when you leave the transitional care unit

Services are billed to your insurance company, including Medicare. We will submit a claim for services on your behalf. Please contact your insurance carrier for questions about your insurance benefits.
