Health care coordination and waiver case management services for people with disabilities

AXIS Healthcare helps people with disabilities maintain health and live well in their communities. We provide health care coordination and case management. We are passionate advocates for people with disabilities.

Without care coordination and case management, many of the people we serve would see a decline in their overall health and safety, be in the hospital more often, or be placed unnecessarily in nursing homes. Our approach is consistent with consumer-directed care which can contain costs, prevent unnecessary waste and avoid duplication of services.

About AXIS Healthcare

We serve a highly diverse population. About 45 percent of our clients are from immigrant or diverse cultures. About 25 percent of our clients have a primary language other than English.

Relocation services

We provide relocation services to people moving out of a nursing home. Waivers provide services and supports that enable people to live in the community. To qualify, you must be eligible for a waiver as determined by your county of financial responsibility.

Health plan care coordination

We partner with health plan providers through Special Needs Basic Care (SNBC) to offer health care coordination. To qualify, you must:

  • have a certified disability
  • be eligible for Medical Assistance
  • be between 18 and 65 years old
  • be enrolled in a health plan that contracts with AXIS Healthcare (UCare, Medica)

County waiver case management

We provide case management through the Community Access for Disability Inclusion (CADI), Brain Injury (BI), Community Alternative Care (CAC), and Developmental Disabilities (DD) waiver for Hennepin, Ramsey and Washington Counties.

Allina Health Everyday Clinic

This is a retail clinic location. We offer basic services for minor injuries and illnesses for patients 18 months and older. We do not see visits for workers' or injury compensation.

Everyday On Wheels

Everyday On Wheels mobile clinic is not wheel chair accessible due to the size of the vehicle. Patients should be able to safely climb a short set of stairs to enter the mobile clinic.

Please arrive 10 minutes prior to your scheduled appointment time and have your mobile phone available for the provider to contact you when they are ready to see you.

We've changed how we provide some services with your care and safety in mind. Expanded virtual care services are available to schedule online. You can also call your clinic for help finding the best option.
