AXIS Healthcare helps people with disabilities maintain health and live well in their communities. We provide health care coordination and are passionate advocates for people with disabilities.
Without care coordination, many of the people we serve would see a decline in their overall health and safety, be in the hospital more often, or be placed unnecessarily in nursing homes. Our approach helps to contain costs, prevent unnecessary waste and avoid duplication of services.

About AXIS Healthcare
We serve a highly diverse population. About 45 percent of our clients are immigrants or represent diverse cultures. About 25 percent of our clients have a primary language other than English.
Health plan care coordination
We partner with health plan providers through Special Needs Basic Care (SNBC) to offer health care coordination. To qualify for health plan care coordination services, you must:
- have a certified disability
- be eligible for Medical Assistance
- be between 18 and 65 years old
- be enrolled in a health plan that contracts with AXIS Healthcare (UCare, Medica)
