Allina Health at Home offers the support, knowledge and assistance to help keep you independent in the comfort of your home. People experiencing chronic and advanced illness often need medical care at home. Allina Health at Home offers many services for you to manage health challenges outside of the hospital.

Types of support we offer

Advance care planning

Advance care planning helps people communicate their wishes for medical treatment. You can do this planning whether you’re healthy or sick. This process will clarify what your family and friends need to know if you become unable to make health care decisions for yourself.

Hospice

Hospice care is for people with a life-limiting illness, meaning people whose life expectancy is determined to be six months or less if the disease runs its natural course. The hospice team focuses on enhancing the quality of life wherever the person calls home.

Home health

Our Home Health team develops an individualized care plan to help you stay independent in your home. Services can include intermittent visits from skilled nursing, home health aides, social services and professional therapies such as physical, occupational and speech therapy. These services will vary depending on your doctor's order.

Service Area: Allina Health Home Health is licensed to provide care in Minnesota. Our service area includes 33 Minnesota counties. 

Home infusion therapy services

Home infusion therapy is for people receiving medication through an intravenous needle or catheter at home instead of the hospital. We deliver medications to you, and a specially trained infusion nurse gives support and education on how to administer the medication.

Mother & Newborn

Some mothers need extra medical care during their pregnancy. An Allina Health Home Health - Mother & Newborn nurse can come to you to provide that care.

Palliative care

Palliative care is for anyone in any chronic or advanced illness stage. Palliative care is focused on improving your quality of life by treating pain and other physical symptoms, understanding and documenting goals of care, and addressing emotional and spiritual concerns. Palliative services are available in several settings: inpatient, in clinics, and the community.

Senior Health

Our Senior Health program gives medical care in transitional care units and gives seniors in-home care through our Complex Care Program. We work with you and your loved ones to provide routine care and manage medical problems.

Home Hospital Care

Home Hospital Care provides acute medical services in the comfort of home. You'll get coordinated care by providers, nurses, and therapists, as well as biometric monitoring and 24/7 access to nurse triage.

Medicare, Medicaid, most long-term care plans and private insurance plans cover many home care services that your health care provider orders.
