What to bring
Make sure to have this information with you:
- name and phone number of your health care agent (the person who can speak on your behalf when you cannot)
- a health care directive form
- A health care directive form will be mailed to you, but you can also access it on allinahealth.org/ACP.
- A notary who can notarize your completed health care directive will be at the class.
